Blood substitutes- the polyheme trials

Sameer S. Apte McGill University, QC, CAN Email: [email protected] ethical debate (1, 6-15). Opponents say that Northfield Laboratories has inappropriately influenced regulatory decisions and abused the bureaucratic system in order to gain approval for a questionable experimental protocol (7, 10, 13). Conversely, proponents argue that prolonged suspension of informed consent is a necessity for effective characterization of safety and efficacy of prospective emergency therapies (6, 12, 14). This putative necessity is hinged upon the fact that the majority of patients targeted by new emergency therapies are incapacitated at the time of enrollment (24, 16). What is not debated is that the completion of the pivotal phase III clinical trial for Polyheme is surrounded by a cloud of ethical tension...

Artificial oxygen carriers: a current review.
Artificial oxygen carriers are not blood substitutes. They serve to carry oxygen to tissues and are either hemoglobin based or perfluorocarbon based. Driving the development of artificial oxygen carriers are concerns involving both the safety and quantity of the blood supply. No artificial oxygen carriers are currently approved for clinical use in the United States. Hemopure has been approved f...
متن کاملA Review of Blood Substitutes: Examining The History, Clinical Trial Results, and Ethics of Hemoglobin-Based Oxygen Carriers
The complications associated with acquiring and storing whole blood for transfusions have launched substantial efforts to develop a blood substitute. The history of these efforts involves a complicated mixture of science, ethics, and business. This review focuses on clinical trials of the three hemoglobin-based oxygen carriers (HBOC) that have progressed to Phase II or III clinical trials: He-m...
متن کاملFrom the Editor’s Desk: Ethics and Current Medical Controversies
EDITORIAL Physicians have many responsibilities to their patients first and foremost but also to other patients using the same resources; to their colleagues; to the healthcare system, towards society at large; to their students; and not to be forgotten, to themselves, to maintain one's integrity by respecting one's moral values. While those duties are often prioritized in ethical and professio...
متن کاملHaemoglobin, oxygen carriers and perioperative organ perfusion.
Under normal conditions, only 20-30% of the delivered oxygen is metabolised. In normovolaemic anaemia, the organism reacts with increases in cardiac output and oxygen extraction. Once these mechanisms are exceeded, allogeneic blood transfusions may be administered. However, such transfusions are associated with serious adverse effects and alternatives such as artificial oxygen carriers are bein...
متن کاملEndothelial dysfunction enhances vasoconstriction due to scavenging of nitric oxide by a hemoglobin-based oxygen carrier.
BACKGROUND To date, there is no safe and effective hemoglobin-based oxygen carrier (HBOC) to substitute for erythrocyte transfusion. It is uncertain whether a deficiency of endothelial nitric oxide bioavailability (endothelial dysfunction) prevents or augments HBOC-induced vasoconstriction. METHODS Hemodynamic effects of infusion of PolyHeme (1.08 g hemoglobin/kg; Northfield Laboratories, Eva...
